PATIENT RESOURCES

Our office schedules by appointment and we make every effort to accommodate your request. We will accommodate urgent and emergent situations the same day. We recognize the value of your time and desire to serve you efficiently. Every effort is made to see our scheduled patients on time. If you are unable to keep your scheduled appointment, please call us at least 24 hours in advance. Also, please call if you know you are going to be late. This courtesy allows us to accommodate other patients. We accept patients without referrals but some HMO’s require a referral from your primary care physician before seeing a specialist. If your insurance requires a referral, we will help you obtain it prior to your scheduled appointment.

Call or Text us at 309-888-9800 to schedule an appointment or fill out our online form to request an appointment.

On your first visit to our office, please plan to:

  • Come at least 15 minutes early so you’ll have time to fill out the necessary forms.
  • Bring a list of all medications you are presently taking, as well as any X-Rays, medical records, and diagnostic testing results that you have.
  • Bring your insurance card and a photo I.D.

To save time on the day of your appointment, please fill out the packet that most appropriately describes your visit and bring it with you:

Dr. Li and his staff are highly experienced in managing and treating work injuries. They understand the complexities of such cases and are dedicated to helping you recover from your work injury. Since we have in office X-Ray, MSK Ultrasound and MRI, we can can accurately diagnose your work related injury quickly so you don’t wait to find out what is wrong. We will provide timely medical reports to make sure that your benefits are not delayed. We will work with your adjuster, case manager and in some cases, your attorney, to make sure that you get the care you need in a timely fashion.

Dr Li and his staff also are one of the select orthopedic offices in Bloomington-Normal that offer in-office pharmaceutical dispensing to patients with approved workers compensation claims. This added convenience allows you to be treated for your work injury and leave our office with the medications in hand to help make recovery time as fast as possible. This combined with our in-office Physical and Occupational Therapy, will get you feeling better sooner. Dr. Li knows that early diagnosis and treatment of orthopedic injuries is important to reducing time away from work. He and his staff work together to provide excellent orthopedic care to the injured worker, while keeping the employer, case manager and adjuster updated on the patient’s status.

If you are being seen for a work injury or have been in an automobile accident, we will need the name, address, and phone number of the worker’s compensation or liability insurance carrier, as well as the claim number and name of the adjuster.

Many workers do not understand their rights when they have a work injury. If you have a work injury in Illinois, you are entitled to having your medical bills paid in full by your employer’s insurance company. You also have the right to choose the providers for your care and are entitled to receiving all reasonable and necessary medical care. There is no minimum length of time that you have to work for your employer before you are covered. If you are hurt during your first day at work, you are still entitled to receive this important benefit, even if you had not worked there long enough to qualify for employer-sponsored health insurance. Illinois Workers’ Compensation law allows an injured worker to receive benefits for the medical services of two physicians of choice, in addition to emergency and first aid treatment. This means that the insurance company should pay for the visits to a doctor you choose, and also for others to which that doctor may refer you, in the same chain of referral. You are then still able to pick a different doctor, who can also refer you to other providers. Dr. Li’s office will communicate with the insurance company to help you get the necessary care and in certain cases we may advise you to obtain legal counsel to advocate on your behalf.

We will submit billing claims to your insurance company. Your insurance company will then send payment and an itemization of any patient co-pays, co-insurance or deductibles. We then send you an invoice for these items, for which we expect prompt payment. We recognize that some health expenses are unexpected. If you cannot pay the full amount at once, please be sure to call our billing specialist to set up a payment plan. We also recognize that health insurance can be confusing and our billing staff can help you.
Prescription Renewals
We request that all prescription renewal inquiries be made during regular office hours. This can be done through our website or by phone. When you call please have your pharmacy phone number available to expedite the process. If you are a patient with an approved workers compensation claim and have received medication from us through our in-office pharmaceutical dispensing program, refills can be picked on site. We suggest that you contact us two days prior to running out of medication, to allow time for a prescription to be called in or picked up at our office.
Orthopedic & Shoulder Center offers a patient portal that gives you the patient the ability to request prescription refills, contact our office to schedule an appointment, or pay an outstanding balance by credit card online. These requests can be made any time of day, including hours we are not in office. The portal is easy and convenient to use.
